Prenatal diagnosis

In a small percentage of cases, however, it is possible for the product of conception to be unhealthy or for diseases to develop in the pregnant woman.

Combined test
Circulating Free DNA Test (NIPT)
Genetic investigation that is based on the quantification of fetal free DNA (cf DNA) assigned to specific chromosomal locus. This method makes it possible to detect excess or deficient amounts of selected sequences. During pregnancy, in fact, about 10% of the free DNA circulating in maternal blood is of placental origin (cf DNA). The test can be applied as a screening test as early as 10 weeks for Trisomy 21 (sensitivity 99%) for Trisomy 18 (sensitivity 97%) and for Trisomy 13 (sensitivity 92%) but is not a diagnostic test and, as such, any high risk result must be confirmed through invasive diagnostic tests. The test does not provide a result in 2-4 % of pregnancies and it is very important that the fetal fraction detected by the laboratory is reported in the report in order to assess the reliability of the test.
Villocentesis
Invasive first-trimester examination that provides the opportunity to be able to assess the fetal karyotype and, therefore, make certain diagnoses. It is performed from 10+0 weeks of pregnancy and consists of an ultrasound-guided sampling of small fragments of chorionic villus (fetal side placenta). There is no need to perform some drug preparation, only to observe a short period of rest after the examination. Its risk of miscarriage is estimated between 0.5 and 1 percent, however, if performed in referral centers and by experienced operators, very low.
Amniocentesis
Invasive examination in the second trimester of pregnancy that provides the opportunity to be able to assess the fetal karyotype and, thus make certain diagnoses. It is performed from 15+0 weeks and consists of an ultrasound-guided sampling of amniotic fluid (about 20 cc). There is no need to perform some drug preparation, only to observe a short rest period after the examination. Its risk of miscarriage is estimated between 0.5 and 1%, however, if performed in referral centers and by experienced operators, very low
Ultrasound screening for malformations in the second trimester of pregnancy. This is an ultrasound scan to be performed between 19 and 21 weeks to assess fetal anatomy. It must, however, be remembered that fetal morphogenesis is a developmental event that cannot be captured in an all-inclusive manner with a single ultrasound examination. The patient should, therefore, be informed of the limitations of the method and the different sensitivity of the ultrasound examination to different fetal malformations.
