TYPE OF PROSTHESIS

Knee replacement can be:

  • Partial or monocompartmental prosthesis when prosthetic replacement involves only a portion of the joint and preserves the cruciate ligaments
  • Total prosthesis when prosthetic replacement involves the entire joint, sometimes including the back of the patella

depending on the type of prosthesis will consist of:

  • Femoral metal component lining the lower end of the femur
  • Metal tibial component that sits on top of the tibia
  • Polyethylene intermediate component placed between the metal parts
  • Patellar/patellar component (in cases where the posterior part of the patella is also replaced)

The need for arthroplasty surgery is generally caused by the occurrence of gonarthrosis, a condition that involves degeneration of the cartilage that lines the articular ends of bones with the function of protecting them right at the joint.

GONARTROSIS

Gonarthrosis of means chronic, degenerative pathology of the knee, characterized by wear and tear of the articular cartilage that connects the tibia and femur.

CAUSES

Gonarthrosis is mainly caused by the natural aging of cartilage tissue cells, which undergoes physiological changes in its structure, becoming more fragile.

The presence of predisposing factors, such as excess weight, alterations in the mechanical axis (varus or valgus knee), joint trauma (injury or rupture of menisci or cruciate ligaments) can anticipate the manifestation of this pathology over time because degeneration of articular cartilage results in increased joint friction, which in turn increases cartilage degeneration, and so on.

Progressive destruction of the cartilage layer results in direct bone-to-bone contact causing pain and inflammatory phenomena. The clinical consequence of the reduction of the cartilaginous layer and chronic inflammation is the appearance of swelling and pain, initially of nocturnal onset, which becomes continuous as the disease worsens and is exacerbated by walking. In the more severe forms of the disease, pain-free walking is possible only for short stretches. Another consequence of osteoarthritis is the progressive and disabling reduction of joint motion.
